Loaner Request

Please fill out the following information to request a loaner device.

  • I understand that I am responsible for any repair or replacement costs incurred due to abuse, neglect, theft or loss.
  • I understand that the loaned product must be returned within two weeks of receipt of the repaired device to avoid late fees.
  • I understand that I am responsible to contact Tobii Dynavox when I need to receive a label to return the loaner.
  • I understand that I am responsible for the late fee of $250.00 per week for each week the device is past due.
  • I understand that the return label that will be supplied is solely for the return shipping of the loaner device and that using this return label to return my device for repair may cause very significant delays in the repair process.
  • I understand that not returning this device in satisfactory condition within the explained time frame may void future loan eligibility.
  • I understand that Tobii Dynavox reserves the right to terminate the loan at any time.

Please look for an email from our technical support team members for updates on availability and processing of the loaner request. Loaners are processed weekly. At the time of termination of the loan, please contact Tobii Dynavox technical support to receive a return shipping label for the loaner.
